Individual Psychosocial Rehabilitation Support Services

Description of the Program

An Individual Psychosocial Rehabilitation Support Service provided to people with a mental illness living in their own homes, whether the home is public or private, family or shared accommodation, a boarding house or supported residential facility.  Individual psychocosocial rehabilitation support services provide structured, goal focused and individually tailored rehabilitation and support services at level of intensity and duration appropriate to consumers needs.  The service works in partnership with Specialist Mental Health Services, consumers and their carers. This service replacesThe Metro Access Program that had been operating since September 1996.

Individual Psychosocial Rehabilitation Support Services supports and empowers individuals to:

  • Fully and actively participate in the community including pursuit of social, recreational, and educational activities
  • Access suitable independent housing (private rental, Housing Associations, SAHT)
  • Increase independent living skills through the development and maintenance of general housekeeping skills (e.g., cooking, cleaning, budgeting, shopping)
  • Access appropriate mental health and medical supports and better manager their own health needs.

Eligibility for Service

  • People who have a serious mental health illness and have high and complex needs
  • Require support to develop skills in a range of areas such as managing a house, finances, social intergration, parenting, managing physical and mental health issues
  • Or are homeles, at risk of becoming homeless or are residing in unsuitable housing
  • Are at risk of requiring hospitalisation or relapsing without support
  • Have other conditions such as drug and alcohol issues, intellectual disability and acquired brain issues
  • Must have a Mental Health Services Key Worker who will make the referral.

UCWPA's Individual Psychosocial Rehabilitation Support Services is able to provide up to 900 hours of support per month across the Central Northern Adelaide Health Services region.

Service and Funding Agreement

The Department of Health, Mental Health Unit, have contracted UCWPA to manage the program, currently funded to 30th June 2011.
