Community Aged Care Packages (CACPs) provide client focused services to people requiring a low level of care, helping them to remain independent in their own home.
CACPs provide a coordinated, planned and reliable service that enables clients to maintain control over services provided. These may include:
Wesley House Aged Care Facility manages 39 packages. Indigenous people are given priority for 15 packages.
People requiring Community Care Packages are required to have a current Aged Care Assessment Team (ACAT) approval and are prioritised on a waiting list.
Packages are responsive to changing needs of clients and service plans are regularly monitored to ensure that they are appropriate. Wherever possible, clients are matched with carers who can support their cultural, spiritual and linguistic needs.
Community Care Applications
Community Care Applications are received via Metro ACAT who send copies of approved applications in this area to the Wesley House CACP Coordinator. The applicant is contacted to assess the level of service required and to determine priorities.
When a CACP becomes available, a Service Agreement is negotiated with the client and their carer. Details including rights, responsibilities and the types and cost of services will be determined at this time.
