Residential Aged Care Waiting List

Each UCWPA Aged Care Facility keeps a waiting list for prospective residents.  To apply for a place in one of our residential facilities, please forward a completed application to the facility of your choice.

If you wish to be considered for more than one facility, please also complete the Preferred Sites form (click on link below).

APPLICATION PACK
The 5 Steps to Entry into Residential Aged Care pack provided by your ACAT Assessor includes an Application for Respite Care or Permanent Entry to an Aged Care Home and a Request for Assets Assessment along with general information and fee structure.

Please complete the forms and send the Request for an Assets Assessment to Centrelink or DVA, as per the back of the booklet.  A statement of your eligibility for financial assistance will be forwarded to you in due course.

On receipt, please photocopy and forward to the Aged Care Facility of your choice along with the completed application form and a copy of your ACAT assessment.

KEEPING IN TOUCH
It is important to keep us informed of any changes to your care needs or circumstances. Please contact the facility at least every 2 months to ensure that your application remains current.
