Community Aged Care Packages

Community Aged Care Packages (CACPs) provide client focused services to people requiring low level of care to enable them to remain independent in their own home.

CACPs provide a coordinated, planned and reliable service that enables clients to maintain control over services provided.

Services may include:

  1. Case management ie: liaising with other service providers
  2. Assisting with personal care such as showering, dressing and grooming
  3. Assistance with light housework such as vacuuming, mopping floors and cleaning bathroom areas
  4. Assistance with laundry
  5. Assistance with shopping and bill paying
  6. Transport to appointments

39 packages are managed from Wesley House by the CACP Coordinator.
15 packages have a priority for indigenous people.

People requiring Community Care Packages are required to have a current Aged Care Assessment Team (ACAT) approval and are prioritised on a Waiting List.

Packages are responsive to changing needs of clients and service plans are regularly monitored to ensure that they are appropriate and acceptable for the client.

 

Wherever possible, clients are matched with carers who can support their cultural, spiritual and linguistic needs and are encouraged to participate in choosing suitable carers.

Community Care Applications:
Most applications are received via Metro ACAT who send copies of all approved applications in this area to the CACP Coordinator who then makes contact with applicants to assess level of service need and Waiting List priority.

When a CACP becomes available, the Coordinator negotiates a Service Agreement with the client and their main carer detailing their rights and responsibilities, types and cost of services to be provided.
